Hypoglycemia frequency is measured as Time Below Range (TBR) - the percentage of readings below threshold:
The ADA/ATTD International Consensus targets are: Level 1 TBR <4% for most people, and Level 2 TBR <1%. High-risk individuals (elderly, hypoglycemia unawareness) should target <1% and <0.5% respectively.
Level 1 (<70 mg/dL / 3.9 mmol/L): Requires intervention - eat 15g fast-acting carbs and retest in 15 minutes (the Rule of 15).
Level 2 (<54 mg/dL / 3.0 mmol/L): Clinically significant - requires immediate intervention regardless of symptoms. Repeated Level 2 events may indicate hypoglycemia unawareness.
