This score is based on validated clinical risk factors associated with hypoglycemia in people with diabetes. Each factor independently raises the likelihood of low blood sugar events:

Score = Sum (age>=65: +2) + (insulin: +2) + (hypo history: 0/+1/+3) + (A1c<6%: +2) + (eGFR<60: +2) + (duration>10yr: +1) + (sulfonylurea: +1) + (cognitive impairment: +2)

Why these factors matter: Older adults have reduced counter-regulatory hormone responses. Insulin and sulfonylureas directly lower blood glucose. Prior hypoglycemia blunts awareness of future episodes (hypoglycemia-associated autonomic failure). Impaired kidney function slows glucose production and drug clearance. Very tight control (A1c < 6%) leaves little buffer before levels drop too low.

Recognition symptoms: Shakiness, sweating, rapid heartbeat, confusion, irritability, and blurred vision. Severe hypoglycemia can cause unconsciousness - knowing your risk helps you prepare.
